An identity card or travel document if you are a Czech citizen; a residence permit, travel document or identity card of an EU Member State citizen if you are a foreign national.

Basic information:

Upon written request by a person whose identity has been verified, an extract from the Criminal Records Register relating to that person shall be issued. An extract may also be issued on the basis of a notarised power of attorney to the person’s authorised representative, or to a solicitor providing legal assistance to that person (Section 11(1) of Act No. 269/1994 Coll., on the Criminal Records Register).
