Basic information
A citizen’s place of permanent residence is the address they choose, usually in the place where they have their family, employment, parents or home. A person may have only one place of permanent residence, namely in a property identified by a house number or registration number and intended for housing, accommodation or private recreation.
- notification of a change of permanent residence,
- notification of the termination of permanent residence in the Czech Republic.
- Application for the registration of an address for the service of documents.
Procedure
- When changing their permanent residence, a citizen must complete and sign a permanent residence registration form and submit the necessary documents. For citizens under the age of 15, the change of permanent residence must be reported by their legal guardian or by the individual to whom the child has been entrusted by a court order. For a citizen whose legal capacity has been restricted to the extent that they are incapable of acting, the change must be reported by their legal representative. A member of the household whose authority to represent the citizen has been approved by a court, or a person authorised by them on the basis of a special power of attorney with a notarised signature, may report the change of permanent residence on the citizen’s behalf,
- Upon termination of permanent residence in the Czech Republic, if a citizen decides to terminate their residence in the Czech Republic, they must notify the registration office for their place of permanent residence in writing and surrender their identity card. This notification must bear a notarised signature; this requirement does not apply if the citizen signs the notification in the presence of a registration office employee. The notification may be sent by data message bearing a guaranteed electronic signature or via a data box. An authorised representative may also notify the registration office of the termination of residence on behalf of the citizen, based on a special power of attorney with officially certified signatures;
- an application to register a delivery address must be submitted in paper form at the citizen’s place of permanent residence; it must bear an officially certified signature, unless the application is signed in the presence of a staff member at the registration office. The application may also be submitted via a data message bearing a guaranteed electronic signature or via a data box.
