Request for a statement from the points-based system

Where to deal with this matter: Příbram Town Hall, Transport Office, Gen. R. Tesaříka 19, 261 01 Příbram I, Driving Licence Department, ground floor, on the left, counters 7 and 8 “Driving Licence Register”.

Documents required: Proof of identity and driving licence (if issued).

Procedure: Submit the application in person at counters 7 or 8 of the Driving Licence Department. If the applicant is unable to attend, their representative must have a certified written power of attorney.

Processing time: Usually dealt with whilst you wait.

Who is eligible to apply: Any individual.
