Other necessary documents, information, etc.:
A valid identity document of the person authorised to act on behalf of the legal entity.
Anyone acting on behalf of a legal entity must provide proof of their authorisation (e.g. an extract from the Commercial Register).
In the case of representation by power of attorney, a written power of attorney must be provided.

Basic information:
A licence to establish a gaming area for the operation of bingo, technical games and live games is issued by the municipal authority within whose territorial jurisdiction the gaming area in question is situated (Sections 97–104 of Act No. 186/2016 Coll., on Gambling). In the application, the applicant must specify the type, address and opening hours of the gaming venue; the type of gambling to be operated at the venue; and the type, serial number and number of gaming terminals through which the gambling will be operated; and must attach the following:

(a) a basic licence to operate a game of chance;

b) proof of payment of the security deposit;

c) a certificate of operational suitability for each piece of technical equipment through which the gambling game is operated;

d) proof of the legal basis for the use of the premises in which the gambling activity is to be operated;

e) a diagram of the CCTV system.

The supporting documents must not be more than 3 months old at the time of application. Supporting documents must be submitted in the original or as a certified copy.

Who is authorised to submit an application, notification, etc.: A
person authorised to act on behalf of the company (e.g. a statutory body, an employee authorised by a statutory body, a proxy, or a person authorised by power of attorney).
