Basic information

A spouse who took the other spouse’s surname upon marriage may, within six months of the divorce decree becoming final, notify any registry office that they are reverting to their former surname.

30 days

Who is entitled to submit an application, notification, etc.

A divorced spouse who took their spouse’s surname upon marriage, and a divorced spouse who, in addition to the shared surname, listed their previous surname in second place.

Available at the Příbram Municipal Office, Registry Department.
