Dispensing of opiate prescriptions and request forms

Pursuant to Act No. 167/1998 Coll., on addictive substances (as amended), and the provisions of Decree of the Ministry of Health of the Czech Republic No. 54/2008 Coll. the department is responsible for the printing and distribution of forms for medicinal products containing Group I narcotic drugs and Group II psychotropic substances, namely prescription forms and request forms with a blue stripe (so-called ‘opiate forms’).
An amendment to Act No. 167/1998 Coll., on narcotic substances and amending certain other acts, as amended, and to Act No. 378/2007 Coll., on medicinal products and on amendments to certain related Acts (the Medicinal Products Act), as amended, has come into force, and it is now mandatory to issue electronic prescriptions also for medicinal products containing narcotic substances (previously prescribed on a paper prescription form with a blue stripe).
From 1 January 2022, the Příbram District Authority continues to supply blue-striped prescription forms and blue-striped request forms, provided, however, that a medicinal product containing a controlled substance listed in Annex 1 or 5 of the Government Regulation on the List of Controlled Substances, which is not simultaneously listed in Annex 8 of the Government Regulation on the List of Controlled Substances, may be prescribed by a doctor and dispensed by a pharmacist in a pharmacy only on the basis of:
- a prescription issued in electronic form (hereinafter referred to as an ‘electronic prescription’) marked ‘highly addictive substance’,
- a prescription with a blue stripe issued in paper form, in exceptional cases where the conditions set out in Section 81f of the Medicines Act for the issue of a paper prescription are met; a medicinal product containing cannabis for medical use may not be prescribed or dispensed on a prescription bearing a blue stripe, or
- a blue-striped request form issued in paper form.

The issue of opiate prescription forms by the Příbram Municipal Authority is carried out in accordance with the established PROCEDURE – the procedure is set out HERE.
As part of the process for issuing blue-striped prescription and request forms, the ORDER form template must be used (forms available for download HERE).
If acting on behalf of an authorised person, a POWER OF ATTORNEY must be issued (template documents for a self-employed individual are available HERE and for a legal entity HERE).
It is always advisable for the applicant (the ordering party) for prescription and request forms with a blue stripe to contact the Head of Department, Ms Markéta Filipová, who is responsible for the distribution of opiate forms, before commencing the process of their issue.
