A completed application form (if a decision has been made to waive the remainder of the driving ban, accompanied by a copy of that decision indicating that it has become final) and an extract from the driver’s record card. If the applicant has been sentenced by a court to a driving ban or has been imposed an administrative penalty by an administrative authority consisting of a driving ban for a period of at least 6 months, the applicant is also required to submit the original of a medical report on their fitness to drive motor vehicles, drawn up by a general practitioner, together with the results of a traffic psychology assessment. The applicant must prove their identity by presenting an identity document. If more than one year has elapsed since the date on which the judgment or decision imposing the penalty or the administrative penalty of disqualification from driving motor vehicles became final, the application must also be accompanied by a document proving the applicant’s professional competence. This document must not be more than 60 days old on the date the application is submitted. Where a disqualification has been imposed for a period of 18 months or more, or where an administrative penalty has been imposed pursuant to Section 125c(1)(d) of the Road Traffic Act, the applicant must also provide evidence of having completed a therapeutic programme.
